Name your main position, preferred foot, current club or team and the level you play at. Add a secondary position if it is one you regularly play. Clear, accurate details are more useful than exaggerated claims.
Choose useful footage
If you have match video, choose footage that makes it easy to identify you. Include the date, your shirt number and your position. Check that the link can be opened without requesting access.
Keep it current
Update your experience and contact details when they change. If you have already submitted an application, quote its reference when speaking to the team about a correction.
